Serving the greater Stanislaus County with 313 plus acute care beds, Memorial Medical Center provides a full continuum of care including 24-hour emergency care, inpatient and outpatient surgery, obstetrics, pediatrics, nuclear medicine, dialysis therapy, diagnostic imaging, intensive care, physical therapy, skilled nursing services and home health. Specialized care includes Memorial Regional Cancer Center, Memorial Heart Institute and Medi-Flight of Northern California.

Under the supervision of the Cardiac Independence Program (CIP) Supervisor, the Exercise Physiologist (EP) is responsible for providing a wide range of services including but not limited to:
Assessment, evaluation, and education of adult and geriatric CIP clients to include exercise prescription, heart disease, and risk factor reduction.
Education, supervision, leadership, motivation, and instruction for a wide variety of exercises to include evaluation of exercise capacity and exercise prescription for clients enrolled in Phase II, III, and IV CIP.
Case management of CIP client under direction of the CIP Registered nurse (RN).
Participation in community education and outreach programs, to include public speaking an teaching.

Qualifications
Bachelor of Science degree in Exercise Science or a related health field. American College of Sports Medicine (ACSM) Exercise Specialist or clinical Exercise Physiologist certification or equivalent required. Minimum of one year work experience in a Clinical Preventative/Rehabilitative Cardiac Rehabilitation program. Specific knowledge of cardiovascular disease and disease process, cardiac conditions, interventions and clinical exercise physiology for individuals with cardiac as well as multiple diseases and disabilities. Extensive knowledge of cardiac risk factors and risk factor modification techniques. Knowledge of cardiovascular pharmacology and effects of various medications on exercise performance. Ability to identify signs and symptoms of cardiovascular disease, congestive heart failure, peripheral vascular disease and arrhythmias. Ability to motivate and encourage clients in making heart healthy lifestyle changes. Previous experience in conducting educational programs and counseling addressing the psychosocial and emotional needs of the client with cardiovascular disease. Knowledge of quality assurance, data collection, and outcome assessment as related to CIP client. Ability to provide education, supervision, leadership to adult and geriatric individuals in a professional and courteous manner. Minimum of six months computer/data processing experience. Current BLS certification required. Must successfully complete the ECG monitoring course and/or the challenge exam. Must be able to read 12 lead electrocardiograms and rhythm strips. Must be able to travel in and out of Stanislaus county. If MMC and/ own vehicle used on Association business, must possess valid California Driver's License and upon request proof of liability insurance. Job Description - External:
Responsible for providing expert, professional care in the Cardio-Pulmonary Department by assessing, planning, implementing, evaluating, and providing care in cardiac diagnostics, Cardiac/Pulmonary Rehabilitation, and Health Enhancement programs.

CARDIOLOGY-DIAGNOSTICS:

TREADMILLS:
Explains procedure and prepares patient for test.
Knows and recognizes contraindications for exercise testing and notifies provider if present.
Operates equipment, monitors vital signs, and assists provider as indicated.
Recognizes and responds to adverse patient conditions which may arise during testing.
Responds and participates in emergency situations.
Assesses patient functional capacity based on exercise test as needed to assist provider in advising patient as to return to work status and/or activities of daily living.

HOLTERS:
Applies and removes Holter monitor recorder.
Operates Holter scanning equipment.
Evaluates and interprets Holter EKG and prepares the report to the provider. Recognizes abnormalities in the EKG and knows proper procedure for notifying the provider.

PACEMAKER FOLLOW UP:
Checks via telephone or in the clinic ' recognizes appropriate function, abnormalities, and trouble shooting.
Record keeping ' ensuring current and proper documentation of pacemaker follow up on the pacemaker clinic records. Preparing reports for the providers and medical records.
Reporting to providers regarding patient's current pacemaker status.
Recognizes pacemaker malfunctions and 'End Of Service' indicators. Notifies cardiologist/PCP and assists in scheduling patients for follow up or replacement as indicated.
Enrolls new patients in the pacemaker clinic. Orients patients and families regarding pacemaker function and pacemaker clinic follow up routine.

CARDIAC REHABILIATION:
Evaluates patient for program entrance (interview, medical record review for history, pre-admit testing).
Develops patient exercise program and goal setting.
Assists patients during class: monitoring EKG rhythm, vital signs, observing for adverse symptoms, leading warm ups and cool down.
Responds to urgent or emergency situations.
Prepares, updates, evaluates, and presents patient education lectures and handouts in cardiac rehab.
Recognizes when to progress patient through the program.
Evaluates patient at program mid-point and at graduation giving patient and provider feedback as to progress.
Re-evaluates patient prior to graduation and counsels on home exercise program in conjunction with exercise physiology staff.
Keeps providers updated as to patient's progress.

OTHER DUTIES
Assists with Walking Group as needed.
Assists with Pulmonary Rehab classes as needed.

* Pursuing the ongoing personal development necessary for the effective performance of your role, including the latest medical updates, specifically those that relate to lipid management or heart disease. Sharing new information with peers.

* Contributing to process improvement by assessing client interactions, determining strengths and/or weaknesses of internal support material, and communicating your assessment to the Clinical Consultant Manager.

* Maintaining the BHL corporate image and reputation.

Qualifications:

* Bachelor of Science in nursing. Masters degree, preferred, if possible.

* Clinical experience working with patients in lipid management setting utilizing Advanced lipid testing, preferably Berkeley Testing or other Lipid Tests.

* Self-motivated, results-oriented with a positive outlook.

* A mature demeanor, credible and comfortable interacting with physicians within a clinic setting.

* Must possess excellent face-to-face, email and phone communication skills.

* Must have computer skills.

* Must be able to work from home. Some regional travel may be required.

***Berkeley HeartLab will be interviewing immediately. First you'll have a phone interview, then a face-to-face, and last - a final interview with the National Director.

The Clinical Consultant will be based out of their home office & travel is roughly 30%.
